Quarantine Ordinance

The required documents for petitions regarding “Quarantine Ordinance” (aging, ineligibility/incompetence, dementia/lunacy, disability/illness)



1) Printed application of the applicant (petitioner) includes all the information about the individual to be placed under quarantine ordinance, followed by contact information and address of the applicant (petitioner) in case of the need of any clarifications about any information.



2) In the event that the applicant was not genuine, a power of attorney at law or a certified truly original image replica duly certified, must be submitted. The ratification of the aforementioned document should not exceed three months to the date it was issued when submitted.



3) In the event that individual to be placed under quarantine ordinance is a minor, a Genuine / Original birth certificate must be provided, and if the submitted petition refers to “quarantine ordinance” due to “aging and/or ineligibility/incompetency and/or dementia/lunacy and/or disability/illness”, a duly certified genuine medical report issued by a medical committee of the Ministry of Health must be provided stating the exact medical condition of the individual to be placed under quarantine ordinance.



4) A certificate issued by the parish priest of the Orthodox Church and another by the local competent authorities stating the “Good Conduct” of the petitioner.



5) A valid Image replica (photocopy) of the ID’s and/or passports of the petitioner and of the individual to be placed under quarantine ordinance.



6) Payment of the requested fees according to the fees list issued by the Court.



***NOTE: all the above mentioned documents should be submitted to the Court. Any petition that does not meet the requirements/conditions will be rejected, as the Court may request other documents if and when deemed necessary.
