Name Amendment / Correction

The required documents for petitions regarding “Name Amendment / Correction”

 


1) Printed application of the applicant (petitioner) includes all the information about the Distribution of Inheritance (determination of heirs) in which the petitioner has legal interest to amend and/or correct. The application/petition must include contact information and address of the applicant (petitioner) in case of any needed further clarifications.

 


2) In the event that the applicant was not genuine, a power of attorney at law or a certified truly original image replica duly certified, must be submitted. The ratification of the aforementioned document should not exceed three months to the date it was issued when submitted.

 


3) Duly certified Image replica (photocopy) of the Distribution of Inheritance certificate of which the petitioner desires the “Name Amendment / correction”

 


4) Genuine / Original certificate issued from the parish priests of the petitioner/applicant who has legal interest in “Name Amendment / Correction”, stating the individual to be named according to the correct names to be amended and/or corrected.

 


5) Genuine / Original certificate issued by the Municipality and/or the Local Council and/or the Ministry of Interior Affairs of the origin place of birth of the petitioner stating  the individual to be named according the correct names to be amended and/or corrected.

 


6) A valid Image replica (photocopy) of the petitioner/applicant ID.

 


7) Payment of the requested fees according to the fees list issued by the Court.

 


***NOTE: all the above mentioned documents should be submitted to the Court. Any petition that does not meet the requirements/conditions will be rejected, as the Court may request other documents if and when deemed necessary.
